Step 1: Click on the form icon at the top of the page.  This will take you to our registration packet.  (This years packet is 6 pages long, the reason for this is because the packet includes information for Nationals.  All 6 pages must be filled out in its entirety!!!)

Step 2: Print out the registration packet , fill out the packet entirely, bring packet to registration.

***NOTE*** It would be beneficial to both the participant and the Berwyn Youth Football and Cheer Board if the entire packet is turned in together at registration.  This would include page 3 the "Medical Clearance Form", which needs to be completed by your child's doctor.***

**We have now added a sponsorship form for anyone who is interested in sponsoring our league. **

If you or a company you know would be interested in sponsoring our league for the 2010 season, please click on the icon for the sponsorship packet, print it, complete it, and mail it back to us or give to a board member during the season.
